With physician services, your loved one will receive quality health care in a setting where he or she calls home. As an interdisciplinary team of professionals, we all work together with the patient and the family to establish and update the patient’s plan of care. Our staff always keeps the patient’s physician informed of all his or her needs.
Our physician will:
- Act as the patient’s director of care
- Oversee your loved one’s clinical aspects of hospice care
- Provide medical education to the family and the care team
- Participate in admission, eligibility, and recertification of hospice patients
