A team of trained professionals helps design a specialized plan for each patient’s pain management and symptom control. This interdisciplinary team includes:
- A physician
- A hospice physician or a medical director
- Nurses
- Home health aides
- Social workers
- Bereavement counselors
- Spiritual counselors
- Trained volunteers
- Speech, physical, and occupational therapists (if needed)
Hospice care occurs wherever a person calls home – a residence, a nursing home, an assisted living facility, or an inpatient hospice home. Most patients prefer staying at home, but sometimes situations arise and a higher level of care, inpatient care, is necessary.
